Modern management of anterior fossa meningiomas

This comprehensive review synthesizes over 150 studies to tackle the unique surgical challenges of skull base meningiomas. With tumors often reaching massive sizes (mean 5.4 cm) before diagnosis, the paper provides a vital framework for neurosurgeons—emphasizing that psychiatric symptoms (seen in up to 70% of cases) must not be overlooked. It delivers a critical, evidence-based comparison of pterional, bifrontal, and endoscopic approaches, highlighting the 90% gross-total resection rate against the backdrop of complication trade-offs like cerebrospinal fluid leaks, which can reach 40% in endonasal surgeries.

While gross total resection remains the standard of care, modern management demands a tailored approach based on radiological anatomy, tumor consistency, and vascular encasement. The integration of molecular insights—linking TRAF7 and AKT1 mutations to anterior fossa locations—marks a new era in predicting tumor behavior and optimizing surgical outcomes in resource-limited settings.
